Specialized Rehab Unit

Our Transitional Rehab Unit is dedicated to short term rehab to home stays.  Private and Semi-Private rooms are available, all with flat screen televisions, wireless internet, and phone service.  We have a fully staffed therapy department that provides physical, occupational, speech, and aquatic therapy.  Therapy services are available 7 days a week.  In addition to therapy we can provide:  wound care, I.V. therapy, oxygen therapy, medication management, and many more services.  Residents on the rehab unit are seen daily by a nurse practitioner.  Medication regimen is monitored by our qualified pharmacist.  When it is time for discharge our pharmacist will make a special visit to resident and caregiver to provide education on home medications.  Our unit coordinator works with residents and caregivers throughout the rehab stay to ensure a positive experience.  We will set up home health or out-patient therapy services at time of discharge, as well as order any needed home equipment.  Following discharge, our unit coordinator will follow up with residents at home to ensure all needs are being met. 

 

Autumn Court

Autumn Court is a secure unit designed to provide care to those with Alzheimer’s Disease or those that tend to wander.  Life in Autumn Court is centered around a dining and activity area, rather than a nurse’s station, and provides a peaceful home like environment.  We offer specialized activities to residents including:  small group activities, music and pet therapy, supervised outdoor activities in our secure courtyard, and specialized one-on-one activities. 

 

Health Facilities Rehab Aquatic Center

Aquatic therapy allows patients to begin therapy sooner and enhances post-surgery recovery.  The healing properties of warm water may lessen pain associated with arthritis, fibromyalgia, and spinal/joint injuries.  Aquatic therapy can benefit those suffering from back or joint pain, joint replacement, Parkinson’s Disease, stroke, head injuries, balance disorders, or sports injuries.  Our pool includes a variable speed underwater treadmill and underwater camera system to monitor and measure progress.  Aquatic therapy is available during in-patient stays as well as on an out-patient basis. 

Questions and Answers


Q. What can I bring with me?

A.  For your semi-private room, you could bring your own TV, and possibly a favorite chair.  Add your own decor of pictures, table lamps, etc.  Private rooms can accommodate more things.  Remember, this is your home and we want it to be as homelike as possible.

Q.  What are your visiting hours?

A.  We are a 24 hour facility.  Visitors may come to see you anytime during the day or evening.  Visitors should be mindful of your schedule to make sure you are avaliable for company.

Q. What about telephones?

A. You can have a bedside phone in your room, arrangements can be made. 

Q.  Can my regular physician or specialist continue to care for me?

A.  Of course.  For those who don't have doctors that will follow them, our Medical Directors will see that they have such care if and as it is needed.

Q.  What about cost?

A.  There are various payment methods accepted.  Our Business Office Manager and Social Service Director are available to assist you with your options.
